We hope you will take this opportunity to have us prepare your estate plan. To assist in the preparation of your documents, we request that you review and complete this preliminary questionnaire. We have also listed various documents that would be helpful for you to provide for our review, if available.

The personal information questions are designed to elicit information concerning assets, liabilities and individuals for whom you may have an obligation or desire to provide for support. Also listed in the questionnaire are some considerations as to dispositive provisions that you may wish to consider prior to our meeting.

Please remember, it is not necessary for you to fill in every blank nor should you feel obligated to provide all of the financial or other information requested. The purpose of this form is to help with the preparation of your documents, and to prompt your consideration of certain issues related to your estate plan. It is not a homework project! Please spend no more than 15-30 minutes completing this questionnaire. We can obtain additional information from you as necessary. Please be assured that all information provided will be kept strictly confidential as part of the attorney-client privilege.
